Insight Hospital and Medical Center issued a substitute notice stating it detected unusual network activity in September 2025 and determined an unauthorized individual accessed its network between August 22 and September 11, 2025. The notice stated affected individuals would be notified after completion of a file review and listed potentially involved data types, including identifiers (name, SSN, DOB), government IDs, financial account information, and treatment/insurance-related information.
